You may be eligible to join SHIP
. [f you are resident of Massachusetts regardless of age
. you have a document of externally cause of traumatic brain-injury
. You have a related issue with cognitive physics and/or behavioral area.
. You are able to participate in community bas services.
How To Apply for SHIP services Anyone can contact SHIP by phone or e-mail and request a Application
After an application is received SHIP determines eligibility by reviewing all available records related to the Individual brain-injury. Gathering the records take time if you already have records of the initial injury it will help if you send us copies but this is not all required You will get a letter telling you whether you are eligible. SHIP staff can descuss the process with you and aanswer your question about services and can help you fill out the applications.
